• Understand that semi-permanent makeup is a cosmetic procedure involving skin-penetrating pigment and carries risks (e.g., infection, allergic reaction, pigment migration).
• Understand that multiple sessions may be required for optimal results and that results cannot be guaranteed.
• Accept responsibility for following all after-care instructions and attending the scheduled touch-up session.
• Authorise ZeZe Beauty Art to perform the treatment selected and release liability except where required by law.
• Confirm that I am aged 18 or over and have the legal capacity to consent.
